Flexi-Contributions Application Form
Set up a regular Flexi-Contribution account by monthly direct debit and/or lump sum deposit.

After we receive your form, we will contact you for the purpose of due diligence prior to the completion of your application. You are recommended to call our member hotline on 3128 0128 or visit any of our designated branch to learn more about this requirement beforehand.

Change of Investment Instruction Form
Change the investment instruction of your existing investments and/or future contributions and/or accrued benefits transferred from another Registered Scheme. This form is applicable if you want to:

• Change the investment allocation of your existing investments, future contributions and accrued benefits transferred from another Registered Scheme. Appropriate units in your account will be redeemed (if applicable) in order to purchase other funds to meet your new investment allocation percentage; or
• Change the investment allocation of your future contributions and accrued benefits transferred from another Registered Scheme only. The investment allocation of your existing investments will remain unchanged; or
• Change the investment allocation of your existing investments only. The investment allocation of your future contributions and accrued benefits transferred from another Registered Scheme will remain unchanged; or
• Exit the Default Investment Strategy (DIS). All of your existing investments, future contributions and accrued benefits transferred from another Registered Scheme will invest per the investment allocation immediately before you exit the DIS. For the avoidance of doubt, in such case, no subsequent rebalancing or de-risking of the investment allocation will be done with regard to existing investments, future contributions and accrued benefits transferred from another Registered Scheme.

Statutory Declaration on Cessation of Employment

Supporting document for employee's cessation of employment and the date of cessation. This form is applicable in the circumstances where:

• The trustee concerned is satisfied that the former employer cannot be located; or
• The employer concerned refuses to submit a written notice to the trustee about the cessation of the employment of the employee.

Statutory Declaration on reason on cessation of employment and Severance Payment/Long Service Payment

Supporting document for employee's cessation of employment and the date of cessation. This form is applicable in the circumstances where:

• The employee concerned has accrued benefits attributable to the employer's voluntary contributions and/or ORSO transfer; or
• The employee concerned has completed at least 2 years of service with the employer.

This form should be submitted together with the Statutory Declaration on Cessation of Employment (MPF(S)-C(SD)).

Claim Form for Payment of MPF Accrued Benefits (Benefits) on Grounds of Attaining the Retirement Age of 65 or Early Retirement [Form MPF(S) - W(R)] (Fillable format available)

Account Statement Requisition Form

1.    (Applicable to employer or member) Request a copy of any previous employer or member statement for up to 7 years prior.
The handling charge is HKD50 per scheme financial year for each copy.
2.    (Applicable to employer) Request a Member Contribution History Report.
3.    (Applicable to employer) Request for provision of information of employees’ accrued benefits for the purpose of processing the refund or calculation of long service payment/severance payment paid or payable to the related employee for the purpose of Section 12A of Mandatory Provident Fund Schemes Ordinance.
